The NSW Pet Registry makes it easy for you to manage all your pet responsibilities online. This includes recording pet microchip numbers, transferring ownership of pets, updating your details and registering pets. However, in certain cases you may need to submit forms and other documentation to your council.
When you download, print and complete these forms, please share copies with anyone else involved in the process so you can all keep them for your records.
Forms
Identification and microchipping
Permanent identification P1A (PDF, 146 KB)
- Use this form to record microchip numbers. Only vets and authorised identifiers can complete this form.
Verification of existing microchip M1 (PDF, 152 KB)
- Use this form to verify existing microchip numbers. Only vets and authorised identifiers can complete this form.
Registration
Pet Registration R2 (PDF, 147 KB)
- Use the form to register your pet and pay the registration fee. The pet owner must complete this form.
Change of owner
Change of owner by animal welfare organisation or council pound C3B (PDF, 153 KB)
- Council pounds and approved rehoming organisations can use this form to transfer ownership of a pet.
Change of owner C3A (PDF, 150 KB)
- Use this form to transfer ownership of a pet. The person selling or giving away the pet must complete this form. If this doesn't happen, the new owner may complete this form.
Update your details
Update your details C3C (PDF, 118 KB)
- Use this form to change your contact information, home address or postal address. Only pet owners can complete this form.
Annual permits and additional payments
Non-desexed cat and dog fees form (PDF, 143 KB)
- Owners can use this form to pay an additional fee for non-desexed dogs over 6 months old, or an annual permit fee for non-desexed cats over 4 months old.
Restricted or dangerous dog annual permit (PDF, 145 KB)
- Dog owners can use this form to pay the annual permit fee for dogs declared to be dangerous or restricted.
